WHS Incidents Notification Procedures



This WHS Incidents Notification Procedures template sets out the procedures required to be undertaken by the worker, the site supervisor, the worker’s line manager and the HR department when a WHS Notifiable Incident occurs. This template applies to all states and jurisdictions that have enacted the model WHS Laws. For further guidance, see the Guide to Work Health and Safety Laws.

Purpose

The purpose of these WHS Incidents Notification Procedures is to set out the procedures required to be followed by a worker who is involved in a WHS Notifiable Incident as well as [the site supervisor,] the worker’s line manager and the HR department.

The design, planning, implementation and monitoring of the policies and procedures of all WHS obligations of [Organisation name] are part of the policies and procedures described in the following documents:

These WHS Incidents Notification Procedures should be read in conjunction with the following:

  6. Recover at Work Policy

Scope

[Organisation name]’s WHS obligations extend to all workplaces in [business locations] and to all workers of [Organisation name]. The WHS obligations also apply to all current and future activities of [Organisation name], and to any new opportunities [Organisation name] may encounter from time to time.

[Organisation name]’s workers include all of the following:

  1. Employees
  2. Contractors
  3. Sub-contractors
  4. Employees of contractors or sub-contractors
  5. Employee of labour hire company assigned to work in the person’s business or undertaking, or
  6. Outworker
  7. Apprentices and trainees
  8. Students gaining work experience
  9. Volunteers
  10. Any other individuals who carries out work in [Organisation name].

[Organisation name]’s workplaces cover any place where work is carried out for [Organisation name] and any place where a worker goes or is likely to be while at work.

Places include any vehicle, vessel, aircraft or other mobile structure and any waters and any installation on land, on the bed of any waters or floating on any waters.

WHS notifiable incident

Any worker who suffers an injury which is classified as a WHS Notifiable Incident under WHS Laws must comply with the procedures set out below. The Procedures must also be complied with by the worker’s line manager and the HR department.

A failure to comply may result in disciplinary action against the individual concerned.

In the event that the incident is a non-WHS Notifiable Incident, the procedures in this document must also be complied with except only for the notification obligation to the WHS Regulators.

Procedures

WHS Incidents Notification Procedures that must be complied with when a WHS Notifiable Incident and any other type of incident occurs are as follows:

Incident management

Obligation Owner Procedure
Management of incident [Site Supervisor] The [Site Supervisor] has the responsibility for managing the incident until all obligations and procedures set out in this table under which the [Site Supervisor] is the owner have been discharged.
Administer first aid [Site Supervisor] The [Site Supervisor] must ensure that the First Aid Representative or any other workers who was with the worker at or immediately after the time of the incident who is trained in First Aid must proceed to administer first aid and provide care to the injured worker in accordance with the First Aid Policy.

 

Names of current First Aid Representatives are listed in the Contact Details List located at [insert name of document repository].

Call emergency services [Site Supervisor] The [Site Supervisor] must ensure that emergency services are called for immediately (where required).
Preserve incident site [Site Supervisor] The [Site Supervisor] must ensure that the incident site is preserved until a representative from the WHS Regulators instruct otherwise.
Record details of witnesses [Site Supervisor] The [Site Supervisor] must ensure that the names and details of at least one and up to 5 witnesses immediately as soon as the above obligations have been carried out.
Record details of incident [Site Supervisor] The [Site Supervisor] must ensure that the details of the incident are recorded in the WHS Incidents Register within 24 hours of the occurrence of the incident.
